Referring a Patient
To order a sleep study through Midwest Center for Sleep Disorders, please fax us at 630.375.9909 and indicate the following:

• Requested test
• Patient information
• Insurance/referral information
• Brief history

Our staff will contact the patient and schedule the indicated test. Your office will be notified of the test via a fax confirmation letter.

Following the testing, you will be provided with a written preliminary report the morning following testing and a transcribed final report within 3 days. The expediency of test results reporting significantly decreases the overall time involved from the point of the initial order for testing and the physician’s receipt of test results.

The final report includes: test results with normative data comparison, plus impression and recommendations as well as sleep history and a brief medical/surgical/social and psychological history.
