Insurance Coverage 

Midwest Center for Sleep Disorders (MCSD) accepts Medicare, TriCare and many Health Maintenance Organizations (HMO), Point of Service (POS), and Preferred Provider Organizations (PPO).  We are also an  in-network provider for a large number of insurers; please check with your insurance provider to assure that Midwest Center for Sleep Disorders is within your network.

Many insurance providers require pre-authorization for sleep studies. This pre-authorization will be obtained either by your physician, or by MCSD. We will not schedule your study until authorization has been obtained.  If you are insured by a Health Maintenance Organization (HMO), a valid referral is required. This referral must be obtained from your primary care physician.

We do our best at MCSD to verify insurance coverage and requirements prior to testing. However please understand that it is always the patient’s responsibility to assure that all insurance requirements are met. If you are unsure of your coverage, please contact your insurance provider.

Billing and Out-of-pocket Expenses

Payment for services provided are the responsibility of the patient.  At the time of service, please make sure that MCSD has your accurate, complete, and up to date insurance information. MCSD will submit the bill for services directly to your insurance provider.  And if you have more than one, we will also bill your secondary provider.

Out-of-pocket expenses vary by provider, policy and deductible. Please contact your insurance carrier if you have questions. The sleep lab does not have this information, and cannot tell you exactly how much a study may cost you.

If, for any reason, coverage is denied, or if we receive a partial payment for your test, a patient statement will be sent to your home. This amount may include any costs denied or not reimbursed by your insurer, a co-pay or a deductible as required by your insurance provider.
